The 69th Guards Motorized Rifle Division (military unit 02511) from Leningrad Oblast was created in 2024 based on the 138th Separate Motorized Rifle Brigade, which fought in Kharkiv Oblast from the first days of the invasion in February 2022 and subsequently participated in battles on the Kupyansk, Vovchansk, and Slobozhanskyi directions.

The 69th Guards Krasnoselskaya Order of Lenin, Red Banner Motorized Rifle Division was formed in May 2024 by transforming the 138th Separate Guards Motorized Rifle Brigade into a divisional formation of the 6th Combined Arms Army. The division's predecessor, the 138th Brigade, was transferred to Kharkiv Oblast in February 2022 and immediately engaged in combat with Ukrainian Armed Forces formations; in 2023, it conducted an offensive on the Kupyansk direction. After being expanded into a division, as of early 2025, the formation was engaged in battles on the Vovchansk and Slobozhanskyi directions in Kharkiv Oblast.
